Your health will only decrease as the result of a fight. The degree to which you were outmatched in the fight will determine how much health you lose. If you do not pick a lot of fights, you will not have to worry too much about this. You will still want to keep an eye on it though, as others will often pick fights with you. Depending on the circumstances, you can let your health gradually refill over time, go to the hospital and pay cash to have it refilled, or buy a refill from the Godfather with reward points.
Energy
The system will reward you the energy over the time. Your energy level is the important element that will influence on your ability to keep playing and completing jobs. Each job you take on will reduce your energy by a set amount. If you have out of energy then you can do any more jobs. However do not need to worry, your energy level will go back up over time. If you want to be able to keep playing for more than a few minutes at a time though, you will need to do something to increase your energy stores
Stamina
You need stamina if you try to attack other players, rob other players, cash in on a hit on another player, sucker punching, and placing hits on other players. Fighting and Robbing will get in to all the specifics of how you can leverage your stamina, win more fights, get robbed less often and all those great details.
Your stamina will not go down quickly, but if you get into a lot of fights, you will eventually make a dent in it. It also refills over time, and you can buy a refill from the Godfather. Unless you are fighting a lot though, then stamina is something you need to worry about too much.

Choosing your Character type

Since we want to be generating passive income rather than earned and there is no real use for money anymore if you get to around level 40, this is the least interesting of the character types unless your friend group happens to need more eligible Bagmen options.
Maniac
As a Maniac, your energy regenerates at a rate of 1 every 3 minutes instead of 5. At this character, you are also become eligible to be a Button Man which gives you a chance to automatically win any fight you initiate. This was the best type to choose before the Top Mafia changes since energy drives a lot of your leveling. The flip side to this is if you are speed leveling, it counting more on the energy refills from leveling up and Energy Packs than from natural regeneration.
Fearless
As a Fearless character your health regenerates at a faster rate as Fearless Character. Beside of that, you have the ability to be a Top Mafia Wheel Man as well which has the best special ability.
Whenever you perform a job, there is a small chance that job will cost no energy. At higher levels jobs may cost as much as 60 energy to run. This is a very non-trivial bonus and will greatly aid the speed at which you level. But health regeneration itself is not a very meaningful bonus since we can use the hospital to refill health.
Your initial Skill Points
Once you have made your decision, you will get the first skill points to spend. Many others saying that It is highly recommended you just upgrade your Energy till at least 30 before touching the other stats.
How to Star over if you are not happy with the Character
the game does not really support for starting over. If you are unhappy with your initial choices then you have to delete the application and wait 60 days for it to time out. After 60 days, then log in again to get you a clean start in the Mafia world.
